The Manager performs duties consistent with the mission, vision and values of Goshen Health. This position assists the Director of Care Coordination in the planning, directing and overall operations of the department. The incumbent provides direction and leadership to assigned personnel and ensures that all care management functions are performed accurately and efficiently. This position manages ongoing analysis, development and implementation of methods and systems to improve overall effectiveness of Care Coordination processes. Assist in the development of policies and procedures and exists to promote patient advocacy and quality patient care with exceptional clinical and cost outcomes. This position will work collaboratively with Population Health to ensure care across the continuum.
This position is responsible for the consistent assessment, identification and coordination of care provided by care managers during the acute care phase of a patients care.
BSN. Requires and active Indiana license as a Registered Nurse. Master's in Nursing or related health care field is preferred.
Three years clinical experience. Requires clinical skills and an understanding of medical terminology. Knowledge of computer use and various data base software programs. Ability to adapt and be flexible with frequent change. Strong analytical and critical thinking skills. Strong organizational and interpersonal skills.
Previous leadership experience preferred. Requires clinical skills and an understanding of medical terminology, third party payers requirements and reimbursement criteria. Requires ability to apply Intensity of Services and Severity of Illness criteria (InterQual). Knowledge of personal computers, McKesson (Interqual), MIDAS, Meditech. Ability to comprehend third party payer contractual arrangements
